Proaxis Therapy is committed to creating an understandable and reasonable billing process for our patients.

Pre-Authorization

Please be aware that some insurance companies require that we pre-authorize therapy treatments. Our staff will make the calls necessary to accomplish pre-authorization. As always, it is the patient’s responsibility to check for benefits coverage.

We will bill the associated insurance company first. After receiving an Explanation of Benefits from that insurance company, a bill requesting patient payment is sent directly to the patient. We appreciate prompt payment of the patient’s responsibility.

Co-Pays

Proaxis Therapy requires payment of co-pays at the time of service. Please stop at the front desk to settle this balance.
